After completing the tour of a Nursing Home, total all “yes” answers to obtain the Nursing Home’s overall score.


Environment

Is the facility air conditioned?

Is there individually controlled air conditioning?

Is there individually controlled heating?

Is the facility clean?

Is the facility well maintained?

Is the facility free of offensive odours (note: fragrances may be used to disguise offensive odours)?

Are residents’ rooms spacious?

Does the facility resemble a home (as opposed to an institution)?

Is there a separate and private area for visiting residents?

Are residents allowed to bring personal items to decorate their rooms?

Is it easy to maneuver a walker or wheelchair?

Is there a secured outdoor area that residents can use without supervision?

Is there a secured unit for wandering residents?

Does the facility have a secured front entrance?

Does the facility conduct monthly fire drills?

Does the facility have an evacuation plan?


Activities
